The sports physical will be held at BLHS during the school day on April 23. Transportation will be provided. You will NOT be called to get the physical if you do not sign up on this form. All physicals for the 2026-2027 school year must be dated after April 1, 2026. Students will be given a physical form to bring home to get signed once they sign up on the Google Form.
You must return your physical to Mrs. Buzhardt with the History Form completed and signed no later than April 17, 2026.
